Desktop unencrypted Phoebe Putney Memorial Hospital ( PPMH ) held Albany , Georgia has been found wanting. Approximately 6,777 patients are vulnerable after the incident to the misuse of data. Information such as patients' names , dates of birth , addresses , appointments were available services , names of doctors, diagnoses , and social security numbers , on the desktop . Affected patients were seen between May 2010 and October 2013. PPMH sent notification of the incident and an online copy. He offered one year of credit monitoring for affected patients .
PPMH also report the incident to the Department of the Albany Police , but they were not sure about the lost or stolen computer.
